Surgery Referral for Veterinarians

This form will expedite the referral process for patients that you are referring to Humane Colorado Veterinary Hospital for surgery.

Referral information:

Referral does not guarantee that we can accept your patient for surgery.

Same day referrals:

  • Referrals for urgent same day surgery (e.g. abdominal explore, PU) must be submitted before noon. Any submissions received after noon will be reviewed next business day, Monday-Friday.
  • Referrals will be contacted within 2 hours of submission Monday-Friday, except for closure days.
  • Submissions received overnight Monday-Thursday will be called between 7a-8:30am

Non-urgent referrals: Referrals for less urgent/critical cases (e.g. mass removals, dentals) will be contacted within 2 business days.
