By signing below, I hereby accept a position as a Volunteer for Humane Colorado upon the following terms, conditions, and understandings:
Terms and Conditions
Release and Waiver: I understand that the handling of animals and other Volunteer activities on behalf of Humane Colorado may place me in a hazardous situation and could result in injury to me or my personal property. On behalf of myself, and my heirs, personal representatives and assigns, I hereby release, discharge, indemnify and hold harmless Humane Colorado and its directors, officers, employees and agents from any and all claims, causes of action and demands of any nature, whether known or unknown, arising out of or in connection with my Volunteer activities on behalf of Humane Colorado including, without limitation, damages related to bodily injury, personal injury, illness, death or property damage.
Medical Treatment: I hereby release and forever discharge Humane Colorado from any claims whatsoever which arises or may hereafter arise on account of any first aid, treatment, or care rendered to me in connection with my Volunteer activities on behalf of Humane Colorado.
Insurance: I understand that, except as agreed to by Humane Colorado in writing, Humane Colorado does not carry or maintain health, medical, or disability insurance coverage for any Volunteer. Each volunteer is expected and encouraged to obtain his or her own medical or health insurance coverage.
Photographic Release: Understanding that public relations is an important part of a Volunteer’s activities on behalf of Humane Colorado, I hereby grant Humane Colorado a non-exclusive right and license to display, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, adapt, create derivative works, exploit, and otherwise use my name, image, and likeness, and all materials created by or on behalf of Humane Colorado that incorporate any of the foregoing on a perpetual basis throughout the world, and in any medium or format whatsoever, whether now existing or hereafter created, without further consent from or royalty, payment, or other compensation to me, for public relations purposes. I ask that Humane Colorado use reasonable efforts to give me advance notice of any such use, but such notification is not a condition of use for public relations purposes.
